Effect of baricity on spinal anesthesia with bupivacaine


Abstract
Bupivacaine is a potent local anesthetic that is characterized by a long duration of action. This ag; nt has been used widely for epidural blockade and peripheral nerve blocks.
In recent years, considerable interest has been shown in the potential usefulness of bupivacaine or spinal anesthesia. Thus present study was performed to elucidate the effect of-baricity of bupivacaine . dministered intrathecally.
Twenty patients received same volume and concentration. Group I received 3ml of 0.4% bupivacai e and group II received 3ml of 0.4% bupivacaine in 4% glucose. The following results were observed; 1. There were no significant differences between the patients of the twe groups in relation to age, heig t,
and weight.
2. The onset time of maximum segmental spread of analgesia was 22.5¡¾9.50 minutes in group I , d 18.5 + 3.37 minutes in group II which showed no significant difference, whereas there were significa t differences between two groups in maximum sensory spread level. (Group I: T 6.2¡¾ 1.32. Group II: 5.3+0.48. P<0.05)
3. The onset time of complete motor blockade was 23.5 ¡¾ 9.26 minutes in group I and 10.5 ¡¾ 3.69 minute. in group II which showed very significant eary onset in group II. (P<0.01)
4. The cardiovascular changes were observed with no significant difference between the patients of th; two groups.
5. The complications such as mild hypotension in 2 cases, nausea and bradycardia in 1 case were observ ed in group I, but none of the patients developed a post spinal headache or other neurologic symptom. According to this-study, it considered that spinal anesthesia with hyperbaric bupivacaine is useful for lower abdominal, perineal and lower limb surgery.
